Responsibilities

  Conduct patient assessments and develop personalized treatment plans

  Provide therapy to patients with communication and swallowing disorders

• 
  

  Collaborate with interdisciplinary care teams to ensure optimal outcomes

  Document evaluations, treatments, and progress accurately and timely

Requirements

• 
  

  Active SLP license in Arizona (or eligible for licensure)

• 
  

  Minimum of 1 year of SLP experience, with SNF experience preferred

  BLS Certification (American Heart Association)

• 
  

  Strong communication and clinical documentation skills
