As an Occupational Therapist at CenterWell Home Health, you will deliver personalized rehabilitation services to help patients regain strength, mobility, and independence. Your role includes assessing patient functionality, developing therapeutic programs, and coordinating care with healthcare teams, all aimed at improving quality of life.
Key Responsibilities
Assess and screen patient’s daily living/work-related skills
Develop therapeutic retraining programs
Confer with healthcare team members on treatment plans
Provide therapeutic treatment to patients
Review and expand retraining programs
Consult with vocational team members
Report patient status and progress
Participate in care coordination and discharge planning
Design community reintegration activities
Recommend and design special adaptive equipment
Train staff and family members on retraining programs
Supervise Occupational Therapy Assistant and Home Health Aide performance
Document patient care observations and interventions
Required Qualifications
Degree from an accredited Occupational Therapy Program
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
A valid driver’s license, auto insurance, and reliable transportation
Preferred Qualifications
A minimum of six months of occupational therapy experience
Home Health experience
