Job Description
Join Molina Healthcare as a Field Family Nurse Practitioner, where you'll provide essential home and community-based medical care. You'll perform assessments, create treatment plans, and work closely with diverse patient populations to enhance health outcomes in a non-clinical setting.
Key Responsibilities
Provide general medical care and care coordination for various patient levels
Perform comprehensive evaluations and preventive assessments
Address chronic and acute primary care complaints
Establish and document medical diagnoses
Order and perform diagnostic testing judiciously
Create and implement a medical plan of care
Schedule patient appointments and provide post-discharge coordination
Perform in-person visits in various settings including home and nursing facilities
Participate in community-based 'Pop Up Clinics'
Collaborate with fellow nurse practitioners for best practices
Prescribe medications and maintain documentation in electronic medical records
Required Qualifications
Master's degree in family health from accredited nursing program
Active and unrestricted national certification
Current state-issued license to practice as a Family Nurse Practitioner
Current Basic Life Support for Healthcare Professional certification
Current unrestricted driver’s license
Advanced computer skills, proficient with Word, Excel, and Electronic Medical Record
Preferred Qualifications
3-5 years experience as a Registered Nurse and/or Nurse Practitioner
Experience in home health, community health, or public health setting
Previous experience managing chronic conditions in home health
Experience with underserved populations facing socioeconomic barriers
