Your Role

• Conduct comprehensive home health assessments using OASIS
• Create customized treatment plans tailored to patient needs
• Provide hands-on therapy aimed at restoring function and independence
• Maintain precise and timely documentation
• Collaborate with nurses, OTs, and the interdisciplinary care team
• Educate patients and families on safety strategies, mobility, and exercise techniques

What You Bring

• Active (or eligible) Pennsylvania PT license
• At least one year of recent home health experience
• Strong knowledge and accuracy with OASIS documentation
• Excellent communication, organization, and clinical decision-making skills
• A genuine commitment to patient-centered care
