This role is based in a skilled nursing and rehabilitation facility , providing care to a diverse adult and geriatric population.
Assignment Details:
Location: Port Orchard, WA
Start Date: ASAP
Duration: 13 weeks
Schedule: Full-time, Monday–Friday
Hours: 40 hours/week
Setting: Skilled Nursing Facility & Rehab
Responsibilities:
Evaluate and treat residents in accordance with professional standards
Collaborate with an interdisciplinary team to create individualized care plans
Monitor progress and adjust treatment plans as needed
Maintain timely and accurate documentation
Requirements:
Active WA Physical Therapist license (or eligible to obtain prior to start)
Graduate of an accredited PT program
Minimum of 1 year of recent SNF or rehab experience preferred
BLS certification (AHA required)
