Key Responsibilities
Coordinate, implement, and evaluate the home care needs of patients
Review patient insurance information and coordinate activities in care plans
Ensure communication of pertinent findings with family and caregivers
Precept new nurses and supervise peers on field visits
Required Qualifications
Graduate from an accredited School of Nursing
Minimum of 3 years Homecare field experience
NYS RN (Registered Nurse) license
Valid Driver’s license
Preferred Qualifications
BSN preferred
Supervisory or leadership experience
Oasis Certification preferred
Knowledge of Home Health regulations
Ability to supervise, mentor, and coach home care nurses
Computer skills including EMR and Word
