Key Responsibilities
Evaluate member needs via phone or in-home visits
Develop ongoing care plans for high acuity members
Coordinate between members, caregivers, and care providers
Monitor care plans and member outcomes

Facilitate care coordination with providers
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2 - 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
One year of related professional pediatric care experience
