Join MJHS as an On-Call RN, providing essential support to patients and families during critical times. Your expertise will help manage urgent care needs and ensure compassionate end-of-life care.
Key Responsibilities
Provide on-call patient visits for urgent physical and psycho-social issues
Coordinate with Call Center Triage and On Call Manager
Respond to Triage visit requests via cell phone
Communicate with the on-call team and Hospice physician
Implement and educate on the plan of care, including medication management and symptom management
Attend patient deaths to provide emotional support and guidance to family
Report patient-related visits and changes to the plan of care
Required Qualifications
Graduate from an accredited School of Nursing
New York State RN licensure
Preferred Qualifications
At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing
NYS valid driver's license
Willingness to sit for the national board certification CHPN within one year of hire
Proficiency in Word, Excel, and Windows applications
