Job Description
Join MJHS as an Access Center RN Coordinator and make a meaningful impact in hospice and palliative care. In this part-time role, you'll manage complex referrals and ensure safe admissions to our programs, working collaboratively with a dedicated team.
Key Responsibilities
Manage high-tech and complex referrals in the Access Center
Monitor the Access referral list
Work with Access Coordinators to ensure timely admissions to the Hospice program
Communicate with all Hospice Team disciplines including Nurses, Physicians, and Social Workers
Verify all written and verbal orders for evaluation prior to admission
Required Qualifications
Graduate from an accredited school of Nursing
NY State RN License
A minimum of two years Hospice experience
PC Skills
Good written and oral communication skills
Good customer service skills
Preferred Qualifications
BSN preferred
National Board Certification in Hospice and Palliative Care preferred or obtained within one year of employment
