Key Responsibilities
Perform comprehensive patient assessments to determine care needs and transition planning
Lead and collaborate with interdisciplinary teams during daily care rounds and planning meetings
Communicate effectively through written, verbal, and electronic systems
Coordinate discharge planning, including arranging DME, transportation, and insurance authorizations
Manage concurrent utilization review and ensure appropriate resource utilization
Monitor and influence patient length of stay
Understand and apply knowledge of hospital reimbursement and quality measures
Required Qualifications
Active RN license
3–5 years of clinical experience in Medical/Surgical Nursing
Utilization Review
Discharge Planning
BLS certification
Preferred Qualifications
Bachelor’s Degree in Nursing (BSN preferred)
CCM (Certified Case Manager) or ACM (Accredited Case Manager)
Strong interdisciplinary team leadership
Familiarity with discharge planning resources and local community supports
Knowledge of hospital-based care management and payer systems
