Key Responsibilities

  • Perform comprehensive assessments and document findings in compliance with CMS regulations
  • Ensure accurate ICD10 coding and documentation of diagnoses
  • Address and document all quality elements
  • Conduct initial medication review
  • Attend and complete mandatory training requirements
  • Communicate with patients and team members regarding medical concerns

Required Qualifications

  • Certified Nurse Practitioner through a national board for NPs
  • Graduate of an accredited master’s degree in nursing (MSN) program for NPs
  • Board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) for NPs
  • Active and unrestricted license in the state of residence
  • Graduate of an accredited Physician Assistant degree program for PAs
  • Currently board certified by the National Commission on Certification of Physician Assistants (NCCPA)
  • Ability to move a 30-pound bag and navigate various dwelling conditions
  • Availability to work 10 hours per week
  • Access to reliable transportation

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including documentation capabilities
  • Proven ability to develop and maintain positive customer relationships
  • Proven adaptability to change
