This is a contract, Monday through Friday, 5 days a week, 8hr shift days position. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance. No weekends or on call. Area of coverage is Appleton and surrounding areas

Minimum Requirements include:

• Licensed as a Registered Nurse in the state of WI
• Must have at least 1 year minimum of Home Health Experience within the past 3 years
• Hands-on HCHB and OASIS experience required
• Must meet minimum productivity expectations of 30 points per week
