Position: Registered Nurse – Case Manager (RN)

Contract Length: 13 Weeks
Schedule: 40 hours/week (5×8 hr shifts)
Weekends: Every other weekend
Location: Off-site facility (10 miles from main hospital)

Weekly Pay Package: Regular Pay $1,920.00

Stipend $1,700.00

Total Weekly Compensation $3,620.00

Local Pay: $3280.00/week

Job Summary:

We are seeking a highly skilled and experienced Case Manager RN to oversee patient assessments, discharge planning, and care coordination. This role requires working independently at an off-site location while maintaining strong collaboration with the main hospital’s interdisciplinary team.

Key Responsibilities:

• Conduct comprehensive patient assessments to determine care needs.
• Develop and implement individualized care and discharge plans.
• Coordinate care with providers, interdisciplinary teams, and external agencies.
• Perform utilization reviews to ensure appropriate levels of care.
• Accurately document all case management activities in the EMR.
• Advocate for patients to ensure safe and effective care transitions.
• Work every other weekend as per the schedule.

Requirements:

• Active and unrestricted RN license .
• Minimum 2 years of Case Management experience (hospital experience preferred).
• Strong knowledge of care coordination, discharge planning, and utilization management.
• Ability to work independently at an off-site facility.
• EMR proficiency.
