Job Description

Key Responsibilities
Plan, organize, and deliver physical therapy programs
Conduct examinations and evaluations of clients/patients/residents
Document client/patient/resident information for evaluation
Establish and modify therapeutic plans as necessary
Educate clients/patients/residents and their families about treatment plans
Required Qualifications
Licensed as a Physical Therapist in the state of practice
Appropriate education level in accordance with state licensure
Basic Life Support (BLS) certification
Preferred Qualifications
Experience in post-acute, long-term care, or home health care environments
Supervisory experience
