Job Title: Physical Therapist (Adult Rehab Services)

• Location: Oklahoma City, OK
• Duration: 13 weeks (with possibilities of extension)
• Shift: 4X10's Weekend shift will be required
• Guaranteed Hours: 40
• Overtime Starts after: 40 hours

Local Candidates are not accepted (If permanent address is within 50 miles from the facility)

Pay Details:

For Travelers/Stipend:

• Hourly Pay Rate (W2): $80.00
• Weekly Pay Rate (W2): $3,200.00
• Minimum Hourly W2 Pay Rate: $48.85
• Minimum Hourly Stipend: $31.15
• Min. W2 Weekly Gross (Taxable): $1,954.00
• Weekly Stipend (Non-Taxable): $1,246.00 

OR

For Traveller 1099 Candidates:

• Hourly Pay Rate (1099):$85.00
• Weekly Pay Rate (1099):$3,400.00

Requirements:

• Baccalaureate degree in Physical Therapy.
• Current license from the Oklahoma Board of Medical Licensure and
• Supervision as a Physical Therapist.
• Current Basic Life Support issued by the American Heart Association required upon hire.
