Job Description

Key Responsibilities
Provide collaborative care management within hospital and healthcare systems
Coordinate resources among patients, nurses, social workers, physicians, and caregivers
Ensure optimal health outcomes through effective communication and coordination
Support vulnerable populations in a patient-centered environment
Required Qualifications
Bachelor's Degree
California Registered Nurse License
California Fire and Life Safety Card
2 years experience in healthcare related field
2 years experience in Acute Care Management or successful completion of the Transitions in Practice program
Preferred Qualifications
Master's Degree in a Health-related/Science field
