Job Description
Join The Jewish Board as a Case Manager, where you'll support adults with serious mental illness in achieving independence within their community. With a focus on personalized recovery goals and a harm-reduction approach, you will guide residents through their journey to stability and well-being.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Supports residents in medication management independence
Advocates for residents with other service providers
Completes documentation of resident interactions and assessments
Participates in planning and execution of events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
