Job Description
Join Interim HealthCare as a Licensed Practical Nurse (LPN) and enjoy the flexibility of visiting patients in their homes. You will be part of a supportive team providing essential care, while benefiting from generous compensation and outstanding perks.
Key Responsibilities
Participate in ongoing interdisciplinary assessment of the patient
Participate in the development and evaluation of the plan of care
Provide services ordered by the physician as indicated in the plan of care
Recognize and report changes in the patient’s condition to the RN
Counsel and educate the patient and family
Complete timely and accurate clinical notes
Required Qualifications
Current license as a Licensed Practical Nurse (LPN) in the State of MN
At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
Valid driver's license, reliable transportation, and proof of auto insurance
CPR Certification
Preferred Qualifications
2 years experience as an LPN
