Key Responsibilities
Coordinate clinical workflows and processes within physician practices
Analyze patient data and collaborate with healthcare providers
Support clinical staff in implementing quality initiatives and individualized care plans
Identify vulnerable patients for PCMH standards
Develop individualized treatment care plans with physicians
Participate in departmental and organizational committees
Required Qualifications
Current Licensed Practical Nurse license in Connecticut
Minimum of five years nursing experience in a community practice health setting
Experience working with vulnerable populations
Preferred Qualifications
Bachelor's degree in nursing or related field
Experience with PCMH standards
Working knowledge of Microsoft Office products
