Key Responsibilities
Make initial nursing evaluations for hospice eligibility
Identify and reassess patient's/family's needs every 15 days
Initiate and coordinate the plan of care
Document problems, goals, interventions, and responses
Collaborate with the patient/family and healthcare team
Required Qualifications
Current RN licensure in the state of practice
One year of clinical experience
Current CPR Certification
Current driver's license and valid vehicle insurance
