Job Description:

We are seeking a compassionate and skilled LPN/LVN to provide in-home patient care as part of an interdisciplinary healthcare team. This role involves delivering nursing care under the supervision of an RN, following physician orders and the patient's individualized plan of care. The ideal candidate will bring clinical expertise, a patient-centered mindset, and strong communication skills to ensure quality and compliance in all aspects of care.

Key Responsibilities:

• Assist the RN in assessing and identifying patient needs (physical, psycho-social, and environmental).
• Coordinate and implement care plans to deliver effective, cost-efficient services.
• Communicate effectively with patients, families, and team members; document care outcomes accurately.
• Educate patients and families on self-care strategies and monitor their progress.
• Adhere to infection control and OSHA standards to ensure patient and staff safety.
• Support specialized duties, including end-of-life care for Hospice patients.

Requirements:

• Active LPN or LVN license in the state of Oregon.
• Previous experience in home health or hospice preferred.
• Strong clinical documentation and communication skills.
• Ability to float and adapt across assignments as needed.
• Compliance with Medicare, state, and federal healthcare regulations.
