Job Description
Join Alameda Health System as a Palliative Care Program Manager to lead initiatives in palliative care for post-acute facilities. This role offers the chance to collaborate with a dedicated interdisciplinary team and make a significant impact on patient care within a diverse community.
Key Responsibilities
Lead clinical care efforts with an interdisciplinary team
Design and deliver training for facility staff, residents, and families
Manage quality improvement efforts
Consult with other palliative care team members regarding patient progress
Develop and implement new clinical programs and educational sessions
Required Qualifications
Successful completion of an approved Nurse Practitioner training program
Successful completion of a Physician Assistant program
Specific training in palliative care
Bachelor’s Degree
At least 1 year experience in hospice and/or palliative care
Experience working as a Physician Assistant or Nurse Practitioner
Preferred Qualifications
Experience and training in Geriatric care and/or skilled nursing facility settings
Experience working in safety net settings with diverse patient and family populations
Experience in quality improvement and nursing education
