Job Description

Join Alameda Health System as a Palliative Care Program Manager to lead initiatives in palliative care for post-acute facilities. This role offers the chance to collaborate with a dedicated interdisciplinary team and make a significant impact on patient care within a diverse community.

Key Responsibilities

  • Lead clinical care efforts with an interdisciplinary team
  • Design and deliver training for facility staff, residents, and families
  • Manage quality improvement efforts
  • Consult with other palliative care team members regarding patient progress
  • Develop and implement new clinical programs and educational sessions

Required Qualifications

  • Successful completion of an approved Nurse Practitioner training program
  • Successful completion of a Physician Assistant program
  • Specific training in palliative care
  • Bachelor’s Degree
  • At least 1 year experience in hospice and/or palliative care
  • Experience working as a Physician Assistant or Nurse Practitioner

Preferred Qualifications

  • Experience and training in Geriatric care and/or skilled nursing facility settings
  • Experience working in safety net settings with diverse patient and family populations
  • Experience in quality improvement and nursing education
