Key Responsibilities

  • Coordinate individualized patient care from admission through discharge
  • Collaborate with interdisciplinary teams to develop and execute treatment plans
  • Monitor quality of care, service timelines, and resource use
  • Participate in weekly team conferences to review and modify care plans
  • Conduct patient and family education on the rehabilitation process
  • Facilitate discharge planning and ensure continuity of care
  • Maintain accurate and timely documentation
  • Understand and apply knowledge of payor requirements and clinical regulations
  • Attend team and utilization review meetings as needed

Required Qualifications

  • Current license or certification in a healthcare discipline (RN, Social Work, OT, PT, ST, or Rehabilitation Counseling)
  • Eligible for CCM® or ACM™ certification upon entry or obtain within two years
  • RN candidates must hold at least an Associate Degree in Nursing (BSN preferred)
  • Other licensed professionals must hold a minimum of a bachelor's degree (graduate degree preferred)

Preferred Qualifications

  • At least 2 years of experience in rehabilitation or case management
