✅ Qualifications
- Active LPN license in Washington or a Compact License (required)
- Home health experience preferred but not required
- Strong attention to detail and excellent time management skills

- Valid driver’s license and reliable transportation
🎯 Roles & Responsibilities
- Provide skilled nursing care following the active Plan of Care prescribed by physicians or healthcare professionals
- Administer medications, treatments, and therapies accurately and compassionately
- Maintain thorough and accurate documentation of nursing assessments, actions, patient responses, and care coordination
- Educate patients and families on health conditions, disease management, prevention, and self-care techniques
- Collaborate with interdisciplinary teams to ensure continuity and quality of care
- Support patients with individualized 1:1 caseloads, providing personalized attention and care
📋 Requirements
- Flexibility to work full-time or part-time schedules based on patient needs
- Ability to manage a 1:1 caseload with a focus on medically fragile patients
- Commitment to ongoing training and professional growth
- Willingness to travel within the designated counties: Skagit, King, Snohomish, Kitsap, Pierce, Thurston, Grays Harbor
- Strong commitment to patient-centered, compassionate care
