Job Overview
We are seeking a dedicated and skilled Progressive Care Nurse to join our healthcare team. The ideal candidate will have a strong background in patient care, particularly in critical and acute settings. This role involves providing comprehensive nursing care to patients with complex medical needs, ensuring their comfort and safety while collaborating with a multidisciplinary team to deliver optimal health outcomes.
Responsibilities
- Assess, plan, implement, and evaluate patient care plans for individuals in progressive care settings.
- Monitor patients' vital signs and report any significant changes to the healthcare team.
- Administer medications and treatments as prescribed, including IV infusion and acute pain management.
- Perform procedures such as suturing, catheterization, and phlebotomy when necessary.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Collaborate with physicians and other healthcare professionals to coordinate patient care effectively.
- Educate patients and their families about health conditions, treatment plans, and post-discharge care.
- Participate in discharge planning and ensure continuity of care post-hospitalization.
- Provide support for patients with developmental disabilities, memory care needs, or chronic illnesses.
Qualifications
- Valid nursing license with experience in critical care or progressive care nursing preferred.
- Proficiency in electronic medical record (EMR) systems such as Epic or Cerner.
- Experience with cardiac catheterization, dialysis, or ventilator management is highly desirable.
- Knowledge of ICD-9 and ICD-10 coding for accurate medical documentation.
- Familiarity with hospice & palliative medicine, geriatrics, and behavioral health practices is a plus.
- Strong assessment skills with the ability to perform physical examinations and diagnostic evaluations.
- Excellent communication skills to effectively interact with patients, families, and the healthcare team.
- Experience in various clinical settings such as emergency medicine, internal medicine, pediatrics, or outpatient services is beneficial.
- Ability to work collaboratively within a team while also demonstrating independence in patient management.
