Job Description
Join BoldAge PACE as a Primary Care Nurse Practitioner and make a significant impact on the lives of seniors by enhancing their quality of life. In this role, you will provide vital care, coordinate treatment plans, and support a compassionate team dedicated to helping older adults maintain their independence and dignity.
Key Responsibilities
Provide direct and indirect care to participants in the PACE Health Clinic and Day Center
Perform health histories, physical exams, and participant education
Order, interpret, and follow up on lab tests and results
Manage complex clinical situations
Coordinate participant care and make referrals to other healthcare professionals
Participate in the PACE provider call schedule
Document participant changes in the medical record
Evaluate and treat participants in various settings
Required Qualifications
Master’s degree in clinical nursing from accredited Nurse Practitioner program
3-5 years nursing experience (Nurse Practitioner experience preferred)
RN licensure in perspective state
Family certification from Nurse Practitioner program
Experience working with a frail or elderly population preferred
Reliable transportation and valid driver's license
Medically cleared for communicable diseases and up-to-date immunizations
Pass a comprehensive criminal background check
Preferred Qualifications
1 year of experience working with a frail or elderly population
Training on working with frail or elderly population will be provided upon hiring
