A Diabetes and Nutrition Program RN Care Coordinator is responsible for providing comprehensive care management, education, and support to patients with diabetes and nutrition-related health concerns. This role combines clinical nursing expertise with care coordination and patient education skills. The Nurse Care Coordinator will collect and analyze patient-level data, assist with the development and maintenance of diabetes and nutrition care plans, and evaluate outcomes of interventions. Assures compliance with regulatory body standards (including Joint Commission, HRSA, PCMH, and grant funding sources).

Major Duties And Responsibilities

Analytical and Critical Thinking

Assess patients' nutritional and health needs using documentation in the Electronic Health RecordConduct an interview and assess patient’s understanding of diabetes and nutrition knowledge to prepare an individualized care plan.Collect and analyze performance improvement data

Communication

Counsel patients on nutrition issues, healthy eating habits, and diabetes self-managementProvide education on medical conditions, including diabetes, through various formats such as group classes, workshops, and written materialsProvide a basic diabetes and nutrition education and coordinate follow up for Diabetes Self-Management Training (DSMT), nutrition management or pharmacotherapy management.Participate in strategic planning and operational improvement initiativesProvide training and orientation for clinical staff on diabetes care and nutrition managementTrain and supervise diabetes and nutrition navigator to efficiently support the program operations at its max capacity

Teamwork

Collaborate with interdisciplinary healthcare teams to ensure continuity of careCoordinate transitions of care and referrals to internal and external servicesServe as a resource for Chase Brexton staff on nutritional management of patients

Compliance, Policy, and Procedure

Drive compliance to the National Standards for Diabetes Self-Management Education and Support (DSMES)Develop and implement policies and procedures related to diabetes care and nutrition services

Willingness to Learn

Engage in professional development activities to keep abreast of care management practices and patient engagement strategies.Attend training sessions and staff meetings as assigned.

Skills And Abilities

Ability to coordinate communication effectively between various disciplines involved in diabetes and nutrition management.Able to prioritize competing responsibilities and manage complex caseloads.Motivational interviewing skills.Experience with documentation in an Electronic Medical RecordAbility to work with computers in word processing, and database applications preferred.CPR certified by scheduled start date.Must be flexible and adapt to a changing environment.

Education And/Or Experience

Graduate from an Accredited School of Nursing; Current Maryland Nursing LicenseChronic disease management experience, especially in Diabetes.Encouraged to maintain membership in professional organizations such as the AAACN, ANAC, etc.Preferred: Certified Diabetes Educator (CDE) or Certified Diabetes Care and Education Specialist (CDCES)

Working Conditions/Physical Demands

Work is typically performed in an office environment. Depending on client/population needs, may also include clinical and community and/or home-based activities, as appropriate to program.
