Join CenterWell Home Health as an Occupational Therapist and provide personalized rehabilitation care to help patients regain strength and independence in their homes. You'll play a vital role in enhancing patients' quality of life through tailored therapeutic programs and collaborative care.
Key Responsibilities
Assess and screen patient's daily living/work-related skills
Develop therapeutic retraining programs
Administer and interpret diagnostic and prognostic tests
Confer with the patient's physician and healthcare team
Provide therapeutic treatment and instruction to patients
Review and expand retraining programs
Consult with vocational team members for patient plans
Report patient status and progress
Participate in care coordination and discharge planning
Design community reintegration activities
Recommend and design special adaptive equipment
Train staff and family members on retraining programs
Supervise Occupational Therapy Assistant and Home Health Aide performance
Document patient care observations and interventions
Required Qualifications
Degree from an accredited Occupational Therapy Program
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
Valid driver’s license, auto insurance and reliable transportation
Preferred Qualifications
A minimum of six months of occupational therapy experience
Home Health experience
