Key Responsibilities
Provide comprehensive interdisciplinary care coordination for children and adults with chronic life-limiting illnesses
Support service coordination for families to improve health outcomes and prevent unnecessary hospitalization
Educate and train family members and caregivers in self-directed care
Required Qualifications
Current RN licensure recognized by the State of Vermont
Associate's degree in nursing
Completion of a Pediatric Palliative Care Education Curriculum within 1 year of hire
Completion of an approved Grief Counseling Course within 1 year of hire
Current BLS and/or ACLS Certification
Preferred Qualifications
Bachelor's degree in nursing
Two or more years working directly with the pediatric population
Home health/hospice or high-tech nursing experience
