Seeking BE/BC Family Practice Providers to join dedicated team to accommodate growth and additional access to health care for our 11,000 patients.

Group currently has 8 Family Practice Physicians, 7 APRNs, 4 Physician Assistants, 1 Pediatrician, 1 Internal Medicine Physician, 1 Pharmacist, 3 Dentists, 5 Behavioral Health specialists attended by a robust team of clinical and non-clinical support personnel.

As a Patient-Centered Medical Home (PCMH), we prioritize outcomes and integrated care, providing primary care, behavioral health, and chronic disease management to almost 10,000 patients across 26 towns in southern Coos County and northern Grafton counties.
