Join CenterWell Home Health as a Speech Language Pathologist and deliver personalized rehabilitation services to patients in their homes. You'll play a crucial role in improving communication and swallowing functions, helping patients regain independence and enhance their quality of life.
Key Responsibilities
Evaluate, direct and provide speech/language pathology service to patients
Develop and review Plan of Treatment and Plan of Care
Assess and treat disorders of speech, voice, language, hearing and swallowing
Administer and interpret diagnostic tests and therapeutic treatments
Observe, record and report changes in patient's condition
Provide instruction and training in alternative communication systems
Counsel and instruct patients, families, and healthcare staff
Maintain documentation as required by the Company
Participate in care coordination activities and discharge planning
Attend internal staff development programs and obtain continuing education
Required Qualifications
Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
Minimum of six months experience as a speech therapist / speech language pathologist
Current and unrestricted license
Current CPR certification
Good organizational and communication skills
Valid driver’s license, auto insurance, and reliable transportation
