Key Responsibilities
Provide home-based care to patients with illnesses, injuries, and chronic diseases
Work with a team of healthcare professionals to deliver quality patient care
Take patient vitals, do assessments, document progress, and report changes
Assist RN in carrying out the patient's plan of care and teaching self-care techniques
Educate family on the patient’s plan of care, medication, and home safety
Collaborate with interdisciplinary team to coordinate patient care and discharge planning
Required Qualifications
Graduate of an accredited practical nursing program
Active LPN license in Colorado
Minimum of six months of LPN experience
CPR Certification
Knowledge of state and federal home health regulations
Clinical proficiency in nursing
Strong communication skills
