Key Responsibilities
Maintain a safe and healthy home environment
Assist with daily living tasks
Provide support for cooking, cleaning, and budgeting
Medication administration and medical appointments
Daily documentation via web-based application
Provide community inclusion and socialization opportunities
Communicate effectively with team members
Provide transportation
Required Qualifications
Resident of Illinois
Transportation and valid driver's license
Active home/renters insurance
Own or rent home/apartment/condo/townhouse
At least 21 years of age
Reliable computer and internet access
Complete all state mandated trainings
Environmentally safe and clean residence
Ability to pass State/Federal and APS/CPS background check
Registered healthcare worker (DSP)
Preferred Qualifications
