Key Responsibilities
Provide direct clinical services to patients
Coordinate care with medical providers
Screen and assess patients for mental health disorders
Monitor treatment response and symptoms
Support psychotropic medication management
Develop relapse prevention self-management plans
Required Qualifications
Licensed Master Social Worker (LMSW)
Licensed Professional Counselor (LPC)
Licensed Marriage and Family Therapist (LMFT)
3 years of experience treating patients with behavioral health conditions
1 year of experience working in primary care or integrated medical settings
Experience with evidence-based psychosocial treatments
Preferred Qualifications
Experience in the Collaborative Care Model (CoCM)
Detail-oriented and organized
Proven ability to maintain effective relationships
