Department: Case Management General Certification Requirements: RN License, BLS Case load # patients per day/consults per day: 20 EMR: Epic and Care Point for References Do you require InterQual criteria or Milliman experience: No Is this position for UR, discharge planning, or both? Discharge Planning only What areas do they cover Inpatient, Outpatient, ER, Home Care, etc: Inpatient Only Common types of patients & job duties:- Cardiac, Trauma, Oncology, Medsurg Experience or Must have' skills: Discharge Planning CM Manager/Supervisor: multiple supervisors Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) M-F 8a-430p, no self scheduling Weekend Requirements: None Holiday Requirements :None Scrub Color/Dress Code: Navy Blue pants, turquoise tops RT for DC Planning? : No Pharmacy for DC Planning? -: no Department Specific Orientation : can discuss Notes taken: 8/12/2025 MS
