Join MJHS as an On-Call RN, where you'll provide vital support to patients and families facing life-limiting conditions, ensuring compassionate care and effective communication during urgent situations.
Key Responsibilities
Provide on-call patient visits for urgent physical and psycho-social issues
Coordinate with Call Center Triage and On Call Manager
Respond to Triage visit requests via cell phone
Communicate effectively with the on-call team and Hospice physician
Implement and educate on the plan of care, including medication management and symptom management
Attend patient deaths to provide emotional support and guidance to family
Report to triage and primary Hospice team regarding patient visits and follow-up needs
Required Qualifications
Graduate from an accredited School of Nursing
New York State RN licensure
At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing
Preferred Qualifications
Valid New York State driver's license
Willingness to sit for the national board certification CHPN within one year of hire
Proficiency in Word, Excel, and Windows applications
