Job Title: Registered Nurse (RN) - Patient Care Coordinator
Shift: 7:00 AM - 7:30 PM
Position Overview:
The Registered Nurse (RN) will utilize their skills and expertise to deliver high-quality patient care. This role includes patient assessment, care planning, implementation, and coordination of care with various members of the healthcare team.
Responsibilities:
- Assessment:
- Compile databases and formulate nursing diagnoses.
- Utilize physical assessment and interviewing techniques.
- Anticipate interventions needed from other disciplines.
- Determine priorities for patient care needs or problems.
- Reassess patients based on changes in condition or treatment response.
- Planning:
- Initiate and update personalized nursing care plans incorporating scientific and psychosocial principles.
- Coordinate care planning with patients, families, and healthcare team members.
- Implementation:
- Perform clinical nursing skills independently.
- Administer medications, IV therapy, and treatments accurately.
- Initiate emergency procedures as necessary.
- Delegate and coordinate duties of ancillary personnel.
- Document patient status and care plans in medical records.
- Communicate effectively with healthcare professionals and patients.
- Patient Care Coordination:
- Manage daily patient assignments and set appropriate priorities.
- Monitor nursing care delivered by ancillary personnel.
- Teaching and Quality Improvement:
- Develop and implement patient teaching and discharge planning.
- Participate in staff and family education programs.
- Identify and address unit and patient care issues.
- Professional Excellence:
- May participate in RN 3 activities excluding facilitation for the Nurse Residency Program.
- Serve as a charge RN and monitor productivity/patient flow if qualified.
Qualifications:
- 2 years of relevant experience in a similar unit required.
- Current RN license in Maryland.
- Certification in ACLS, BLS, NRP, and NIHSS (must be obtained via NIHStrokeScale.org).
- COVID-19 vaccination is mandatory (declinations not accepted).
- 2 supervisory references (1 from within the last 6 months and 1 from the last 12 months).
Additional Information:
This position may require participation in the on-call schedule and weekend shifts as determined by the facility.
