Key Responsibilities
Utilize patient information sources and direct communications with care team members
Identify patients needing acute inpatient rehabilitation or LTACH
Review patients daily for clinical evidence to request physician order for pre-admission assessment
Required Qualifications
Licensed Professional in a clinical health care specialty (RN, PT, OT, or SLP)
Graduate of an accredited school in clinical specialty
Current state licensure in clinical practice area or eligible for state license transfer
Minimum 2 years recent clinical care experience
Effective verbal, written and persuasive communication skills
Computer and keyboarding skills
Ability to work with others to ensure quality patient care
Organization and time management skills
Strong problem solving skills
Strong interpersonal skills
Preferred Qualifications
Bachelor's Degree, Doctorate Degree, or Master's Degree
Speech Pathologist, Registered Nurse, Occupational Therapist, or Licensed Physical Therapist certification within 30 days of employment start date
