Key Responsibilities
Identify ongoing needs and services of residents
Ensure proper follow through and documentation for changes in health care services
Assist Resident Care Director in conducting resident evaluations
Conduct monthly wellness visits for all residents
Monitor and follow up with residents after falls or incidents
Document all pertinent information in resident wellness files
Communicate with residents' families regarding medical needs
Contact residents' attending physicians as necessary
Ensure weights and vital signs are obtained monthly
Maintain wellness files according to policies and regulations
Provide clinical support to community team members
Understand and follow infection control practices
Monitor each resident's medication and treatment profile
Participate in quality assurance and clinical meetings
Required Qualifications
Graduate of approved college / school of nursing
Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment
Knowledge of nursing practices, techniques and methods applied to health and wellness resident care
Knowledge of good assessment skills
Ability to handle multiple priorities
Effective written and verbal communication skills
Competent in organizational and time management skills
Good judgment, problem solving and decision-making skills
Proficiency in Microsoft Office and Sunrise applications
Ability to work weekends, evenings, and flexible hours
