Job Description
Join Interim HealthCare as an Admissions Visit RN and enjoy independence in providing care to patients in their homes. This role involves comprehensive assessments, care coordination, and skilled nursing duties, with a focus on improving patient quality of life.
Key Responsibilities
Completes admissions and/or visits for the Home Care team
Delivers nursing care as directed by the RN Case Manager and physician's plan of care
Assesses and determines appropriate wound care
Provides accurate and timely documentation of patient services
Determines treatments for the patients
Required Qualifications
Licensed to practice as an RN in Minnesota
Minimum of twelve (12) months experience in acute care or home health
Current CPR certification
Valid driver's license and reliable transportation
Preferred Qualifications
Experience in Home Care including OASIS (minimum one year preferred)
