Join Cooper University Health Care as a Population Health Coordinator, where you'll enhance care coordination through patient-centered approaches. This role involves collaborating with healthcare teams to improve health outcomes and support patients across the continuum of care.
Key Responsibilities
Support and participate in Patient-centered Medical Home (PCMH) concepts of care coordination and team-based care
Assess, plan, implement, coordinate, monitor and evaluate healthcare options and services
Follow established PCC workflow for utilization and self-management support
Participate in PCMH activities including hospital discharge follow-up and care planning
Collaborate with providers to establish mutual goal-setting
Provide outreach, care management, and education for disease self-management
Identify, document, and mitigate patient barriers to improved outcomes
Monitor and evaluate services and community-based resources
Conduct assessments, develop nursing treatment plans, and set goals
Assist team members in developing evidence-based practices and care plans
Collect data on quality metrics for improvement efforts
Participate in process/quality improvement initiatives
Required Qualifications
5 years or more clinical experience
Current NJ RN License
Preferred Qualifications
Experience in primary care
Population or case management experience
BSN preferred
