Job Description
Join BoldAge PACE as a PCP Nurse Practitioner and make a significant impact on the lives of seniors. You'll provide comprehensive care within a supportive, people-first environment, aimed at enhancing the quality of life for older adults.
Key Responsibilities
Perform health histories, physical exams, and participant teaching
Order, interpret, and follow up on lab tests and results
Manage participant care under the supervision of the attending physician
Act as a liaison with healthcare professionals
Participate in the PACE provider call schedule
Document participant changes and communicate with team members
Evaluate and treat participants in various settings
Precept medical students and residents
Required Qualifications
Master’s degree in clinical nursing from accredited Nurse Practitioner program
3-5 years nursing experience with Nurse Practitioner experience preferred
RN licensure in the respective state
Family certification from Nurse Practitioner program
1 year of experience working with a frail or elderly population preferred
Reliable transportation and valid driver's license
Medically cleared for communicable diseases with up-to-date immunizations
Pass a comprehensive criminal background check
Preferred Qualifications
