Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You will be responsible for providing essential client care and developing community resources to support individuals and families in need.
Key Responsibilities
Function as a case planner to ensure appropriate interventions for client care
Develop action plans and utilize existing resources for client care
Conduct routine community contacts and home visits
Evaluate living conditions and provide guidance on independent living skills
Facilitate client referrals to treatment or service providers
Perform individual/group counseling as needed
Manage case documentation and progress notes
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
