Job Description
Join Gentiva as a compassionate Hospice LPN, providing vital hands-on care to patients and families in their homes. Your dedication will help deliver comfort during critical moments of life.
Key Responsibilities
Provide direct nursing care services to hospice patients
Assist the interdisciplinary team in assessing patient/family needs
Report changes in patient conditions to the Registered Nurse
Deliver empathetic care aligned with each patient’s unique plan of care
Serve as a trusted resource to families
Monitor patients and document any changes in condition
Administer medications and treatments according to the plan of care
Participate in infection control practices
Support bereavement activities and closure contacts with families
Participate in on-call duties
Required Qualifications
Graduate of an accredited Practical Nursing program
Current LPN/LVN license in the state of employment
Minimum 2 years of LPN experience in hospice, oncology, long-term care, or home health
CPR certification
Valid driver’s license and current automobile insurance
Preferred Qualifications
3-5 years of LPN experience preferred
Understanding of hospice care principles
Familiarity with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards
