The RN Case Manager is responsible for coordinating patient care to ensure optimal outcomes, efficient resource utilization, and compliance with healthcare regulations. This role involves assessing, planning, implementing, monitoring, and evaluating options and services required to meet an individual’s health needs.
Key Responsibilities:
- Assess patients’ clinical needs and develop individualized care plans.
- Coordinate care across the healthcare continuum, including hospitals, clinics, home care, and community resources.
- Act as a liaison between patients, families, healthcare providers, and insurance companies.
- Monitor patient progress and adjust care plans as needed to achieve desired health outcomes.
- Educate patients and families about conditions, treatment plans, and resources available.
- Ensure documentation is accurate, timely, and complies with legal, regulatory, and organizational standards.
- Facilitate transitions of care and discharge planning.
- Participate in interdisciplinary team meetings and collaborate with providers to ensure continuity of care.
