Job Description
Join UnitedHealthcare as a Foster Care Coordinator, where you will make a significant impact on the lives of children and youth in the foster care system. Utilizing trauma-informed approaches, you will assess and implement care strategies, coordinate services, and advocate for families in need. This role offers flexibility with fieldwork in Reno, Butler, and Sedgwick Counties, KS.
Key Responsibilities
Assess, plan and implement individualized care strategies for children and youth in foster care
Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services
Manage the care plan throughout the continuum of care as a single point of contact
Communicate health-related information to ensure quality coordinated care
Advocate for children, youth, and families to represent their needs and choices
Provide trauma-informed education to members and caregivers
Coordinate with child welfare and juvenile justice systems
Coordinate additional services such as educational advocacy and behavioral health
Required Qualifications
Master’s Degree in Social Work, Nursing, Psychology or related human services area
Unrestricted, independent licensure as a Registered Nurse
2+ years of experience working within the community health setting in a health care role
1+ years of experience working with multi-system children
1+ years of experience working with MS Word, Excel and Outlook
Ability to travel in assigned region
Reside locally to Reno, Butler, Sedgwick Counties, KS
Access to reliable transportation and possess a valid US driver's license
Preferred Qualifications
Licensed Social Worker or clinical degree
Background in managing populations with complex medical or behavioral needs
Experience with electronic charting
Experience with arranging community resources
