Job Description
Join The Jewish Board as a Youth ACT Case Manager, where you'll work with a multi-disciplinary team to support youth facing significant behavioral health challenges. You'll engage families, develop personalized care plans, and connect them with vital community resources to ensure smooth transitions home from higher levels of care.
Key Responsibilities
Provide linkages to community resources and supports
Assist child/youth and family in developing a person-centered care plan
Coordinate health and community services
Engage child/youth and family in defining goals
Monitor, evaluate and record participant progress
Attend interdisciplinary team meetings and mandatory trainings
Complete casework documentation
Required Qualifications
Bachelor's degree required
At least one year of experience working with families and children with Serious Emotionally Disturbed (SED)
Working knowledge of Microsoft Office
Ability to learn electronic health records
Competency in written, verbal and computational skills
