Job Description

Join Danbury as a Memory Care Coordinator where you will lead and implement therapeutic programs for residents with dementia. Collaborate with a dedicated team to enhance the quality of life for residents while enjoying a supportive work family environment.

Key Responsibilities

  • Plan and coordinate therapeutic programs for residents
  • Assess resident characteristics and organize program content
  • Monitor daily functioning of the memory care neighborhood
  • Assist in training and education of staff on memory care programs
  • Develop cooperative relationships and maintain effective communication with staff
  • Market the memory care program and participate in community organizations
  • Maintain accurate and timely documentation
  • Conduct pre-admission assessments for potential residents
  • Assist with resident admissions and transitions

Required Qualifications

  • Must be an LPN
  • Two years of previous experience in programming in assisted living environment

Preferred Qualifications

  • Experience with Alzheimer’s and other dementia individuals
  • 3-5 years of experience in memory care
  • Background in nursing providing one-on-one care for seniors
  • Flexible schedule including availability to work evenings, weekends, and holidays
