Key Responsibilities
Manage daily operations for the Care Coordination department
Develop and implement departmental goals and objectives
Assess quality of patient care and coordinate services
Collaborate on staff education and provide coaching
Monitor departmental budgets and regulatory compliance
Manage staff relations, including performance management and scheduling
Required Qualifications
Bachelor’s degree in Nursing (BSN) or Master's in Social Work (MSW)
Registered Professional Nurse (RN) or Licensed Clinical Social Worker (LCSW) in relevant states
Five years’ experience in case management, with two years in progressive leadership
Preferred Qualifications
2 years experience as a manager or case manager
