Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of clients. In this role, you'll be responsible for developing action plans, providing case management, and connecting clients with resources to support their independence and well-being.
Key Responsibilities
Conduct community contacts and evaluations for clients
Develop action plans for client care
Locate and utilize community resources
Perform home visits and assess living conditions
Provide case management and individual/group counseling
Facilitate referrals to treatment or service providers
Perform community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
